Berdasarkan pemeriksaan Dinas Tenaga Kerja dan Transmigrasi Karawang, diketahui bahwa PT Pindo Deli 2 tidak memiliki standar operasional prosedur terkait penanganan kerusakan jalur pipa pengisian dan tidak tercantum dalam dokumen pengendalian potensi bahaya.
Selain itu, belum teridentifikasi potensi bahaya pada proses perbaikan jalur pipa pengisian.
Sementara berdasarkan temuan Puslabfor Mabes Polri, diketahui bahwa pada saat kejadian alat pendeteksi gas di area pabrik soda kaustik menunjukkan alarm peningkatan gas klor <10 ppm dengan durasi rata-rata ± 30 menit. dan cenderung menurun hingga beberapa jam kemudian.
